This is an exciting new opportunity to join the community learning disability team.
It is a new post which will offer the successful person chance to develop into a band 6 Lead Nurse following a period of preceptorship. We are looking to recruit a recently qualified, or soon to qualify nurse.
We are looking for an enthusiastic and motivated Learning Disability nurse to join our innovative and dynamic team, working for Derbyshire Healthcare NHS Foundation Trust.
The post is based within the community learning disability service, supporting people in community settings.
This post will work alongside colleagues from all professions. The post holder will deliver Nursing provision to a designated caseload, focusing on specific learning disability issues. This will include the assessment, formulation of care plans and evaluation of clinical interventions.
Including a range of individual and group work, and contributing to multidisciplinary team care planning and risk assessments.
Successful candidates will have strong engagement and motivation skills and be able to work well with service users and those supporting them in a broad range of environments. Effective written and verbal communication skills are also essential.
