An exciting secondment opportunity has arisen within the In Reach and Home Treatment Team (IRHTT) to cover for maternity leave. The IRHTT is a developing service which provides support for older adults with functional mental health problems in crisis, to prevent admission and provide care for them within their own homes. The IRHTT also works closely with Ward 1 to ensure that admissions are purposeful and timely, supporting with discharge planning and providing initial follow-up support after discharge where necessary.
Currently the service is Monday - Friday, 9am - 5pm. It is envisaged that over the coming months that this will become a 7 day per week service. There will be an expectation that the successful applicant will be able to work weekends on a rotating basis.
You will be joining a team of highly motivated and skilled staff. The team includes Band 6 Nurses, an OT and Nursing Assistants. As the band 5 within the team you will be expected to complete the 72 hour assessments on the ward, supporting in-patients with periods of leave and planning for discharge, as well as supporting community based patients. You will attend daily clinical meetings, PIPA meetings and be part of ward based MDM's. The successful candidate will be provided with robust clinical and management supervision.
The successful candidate will have the opportunity to contribute to the development of the team and improve the quality of life of those receiving the service.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
