An exciting opportunity has arisen for a Health Screener to join the Derby City Screening Team. This role forms part of the wider 0-19 team and works in close partnership with education, Public Health Teams, and other agencies. The role focuses on the delivery of screening in line with the Healthy Child and Screening Programmes predominately within the school setting.
Derbyshire Healthcare Foundation Trust (DHcFT) are part of an innovative partnership with Ripplez CIC, and Derby Teaching Hospitals Foundation Trust to create Derby City Integrated Healthcare Partnership. The partnership delivers an integrated 0-19 model of care for children, young people and their families.

The post holder will have experience of working with children and young people, be enthusiastic and motivated, have excellent communication skills, and be IT literate and target driven in order to meet the needs of the service. The post holder will work under the Supervision of a Public Health Practitioner and participate in the planning and evaluation of screening activities. They will play a vital part in identifying any concerns with children and young people and support them and their families with referrals to appropriate agencies as required. The post holder will also work as part of a team to maintain records and input data using IT systems.

**Please note this post is based on a term time only contract in order to meet service delivery.**
