An exciting opportunity has arisen within the Chesterfield Central Older Adult neighbourhood for an enthusiastic Band 5 Community Psychiatric Nurse to join their busy and friendly team, based at Bayheath House, Chesterfield.
The successful candidate will be expected to manage a diverse and complex caseload, working in cooperation and developing links with other statutory and independent sector agencies. You will have core skills in assessment, risk management and CPC requirements alongside an excellent knowledge and awareness of CPA. We have strong links with our GP partners and you will be link working into one of our local surgeries.
As a team, we are at the forefront of engaging in the integrated care model being rolled out across Derbyshire to ensure services are wrapped around the individual and as such, a willingness to engage in supporting colleagues in the team and other agencies will be very important. The team are dynamic and innovative in their approach and are committed to providing a high quality service.
This is a very supportive team and you will be encouraged to continue your professional development with the help of regular supervision and access to training opportunities.
You must have appropriate UK professional registration.
