The Dementia Rapid Response Team is a newly formed team that will expand over the coming year to provide home treatment to people experiencing and exacerbation of symptoms associated with their diagnosis of dementia and to those that care for them
This is an exciting opportunity for a dynamic and motivated individual with experiencing of working with people with dementia to join a newly formed team delivering urgent, intense person centred care to people in their own home and prevent hospital admission.
You will be joining a team of highly motivated, skilled staff experienced in delivering care and support to people with dementia. You will contribute to the development of the team and ensure delivery of a service to improve the quality of life of those accessing that service. You will receive regular supervision from experienced senior staff members and will be well supported by the wider team.
The service operates over 7 days, 365 days a year.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
